Radiotherapy planning agents scored 92 to 95 percent decision accuracy but escalated only 25 to 50 percent of the contradictions that required arithmetic, and 24 percent of the plans they submitted missed a hard dose goal

AspireAI community · arXiv (cs.AI)
OpenGray adds radiotherapy planning tasks with known correct decisions to the OpenKBP-Opt head-and-neck dataset, including two contradictions whose dose goals are proven incompatible and a feasible control with a stored plan meeting every hard goal, and evaluates four systems from a rule-based script to a full LLM agent on 30 patients. With Terra, the script preceded by LLM review and the full agent reached decision accuracies of 95.4 and 92.1 percent, yet escalated only 50 and 25 percent of the overlap contradictions, which need arithmetic on the anatomy; all four systems submitted every feasible control, but 24 percent of those plans missed a hard goal the stored plan had met. Logs from 39 stereotactic radiosurgery runs of the clinical agent SAGE show a failure mode that aggregate scores hide: in four runs one goal failed at every attempt, the agent named it in every reply, and it had no action for stopping. The authors' conclusion is that these agents should be evaluated on their decisions as well as their dose, and given an explicit stop-and-request-review action.

THE BIG PICTURE
A FRESH PERSPECTIVE
Research
One distance function matched sparse-autoencoder features across layers and compressed feature circuits into supernodes
Two sparse-autoencoder analyses have resisted scaling: matching semantically similar features across layers, and compressing large feature circuits into interpretable supernodes. This paper argues both are the same underlying problem, estimating semantic distances between SAE features that sit on different activation manifolds. Its representation treats each feature not as a single decoder vector but as an activation-weighted distribution over the hidden states expressing it, projects those distributions into a shared reference space, and compares them with Wasserstein distance. The representation is proved invariant to activation rescaling and stable under perturbation, and recovers true matches under finite-sample margin conditions. Empirically it outperformed decoder-vector and LLM-based baselines and captured subtle functional distinctions between related features, and it compressed large feature circuits into interpretable supernodes automatically. The limitation is that this is interpretability tooling: the paper demonstrates better feature matching and circuit compression, not that downstream interpretability claims become more accurate.
